Milhous was born in Philadelphia, Pennsylvania in 1894. She studied art at the Pennsylvania Academy of the Fine Arts and later at the Art Students League in New York City. She began her career as an illustrator for magazines and newspapers, but she soon turned to writing and illustrating children's books.

Milhous's books are known for their vibrant illustrations, heartwarming stories, and timeless themes. She has received numerous awards for her work, including the Caldecott Medal for The Egg Tree in 1951.
